With a better angle, John can take in the state of Will's clothing better. It really, truly is a lot of blood. And the way he phrased "wouldn't be truthful" makes it seem like some of it isn't his. So then he was in a fight before coming here? Arrived injured and was taken care of in the hospital? Things to think about later. Just because someone appears unassuming doesn't mean anything in this place; for all he knows this Will has some sort of supernatural powers and doesn't need a weapon like the gun John keeps out of sight.
